• You are aware that no research will begin until payment has been received along with your Request Tracking Number which is provided after this form is filled out.
• That geneaology is for sharing. Materials found during your directed research may be shared with other customers, geneaologists, or researchers.

This form is used for Basic Services only. All Additional Research is negotiated over the phone or via email.

The web server will issue you a unique Research Request Number and an on-screen coupon which should be printed and sent along with a $100 check (covering the first 4 hours of research) so research can begin as soon as possible. Please ensure you have entered a valid email address above. A Web URL will be emailed to you so that you can access your Request Coupon at a later date if a printer is unavailable at the moment.
